Medical Insurance:
- 100% Company paid for employees only
- PPO network
- 90/10 Coinsurance
- Vision – One exam per year
- Prescription Drug benefit
Dental Insurance:
- 100% Company paid for employees only
- 80/20 for minor procedures
- 50/50 for major procedures
- Adult and Children Orthodontic
Life/ADD Insurance:
- Company paid Life/ADD
- Company paid Life/ADD insurance for spouse and children
Voluntary Life/ADD Insurance:
- Additional Life/ADD insurance can be purchased for employee, spouse, or children
Long-Term Disability:
- 100% Company paid
- 60% base monthly earnings
Short-Term Disability:
- 100% Company paid
- 60% of basic weekly earnings

- Calendar Year Deductible – None
- Coinsurance –100% covered
- Physician co-pay – $15.00
- Hospital Care - Confinement Deductible – None, Inpatient - 10% covered, Outpatient - 100% covered
- Prescription Drugs – Reimbursement Only - 80% covered
- Emergency Room – 100% covered
Dental:
- 10% Company paid for employees only
- Annual Maximum – $1,500 per person
- Preventive Services – 100% of eligible expenses
- Basic Services – 80% of eligible expenses
- Major Services – 50% of eligible expenses
- Orthodontia – Child up to age 19 – 50% up to $1,500 lifetime
Life/ADD Insurance:
- Company paid Life/ADD
